Proposers, when you invite a Brother Knight to join the 4^th Degree, and you hand them a Form 4, your task is not complete.  It is only the beginning.  Please see the above Form 4, which is typical of the ones I receive for an Exemplification.  All of the Highlighted lines are blank, but all of them must be completed before the Form 4 can be submitted to the District Master to be considered.

The easy part is the Membership # and Council #, they are on your Membership card.  The New Assembly # is also easy, assuming you are inviting your Brother Knight to join ‘your’ Assembly.  Please help the candidate fill in these lines.  We may not remember our 1^st Degree date, but this is your next step, take the Form 4 to your next Membership Meeting and get your Financial Secretary to sign off, and he has access to get the 1^st Degree Date.

Next the Form 4 should be given to your Faithful Navigator, you remember him, he is in charge of your assembly, the assembly you are inviting your Brother to join.  The Assembly will vote on the candidate and then the Faithful Navigator and Faithful Comptroller will sign off on the Form 4.

The Form 4 is now complete, and it may now be submitted to the District Master for the upcoming Exemplification.  Is this a lot of work for a Proposer, absolutely.  Is your Brother Knight/candidate worth it, absolutely.
